"It is a JCAHO Patient Safety Goal regarding Medication Reconciliation.We are currently evaluating our process and believe we may be able to meet the requirement with the current forms in MUMMS . I will let you know of our progress. If you would like to see the requirement, I can fax it to you, but we may not have to do anything beyond what is already there.
"You had asked for input about the Nursing Home Team Meeting this week. It went very well. The Team Facilitator used the LCD to display the IDT form during the review and each staff member had their computers up to enter their progress notes regarding each Care plan problem. They had completed the updated Worksheet and Referral Agreement/POC prior to the meeting and the Medical Records Clerk had them printed out and ready for the Medical Director to sign during the meeting. It went smoothly. We just brought on our Ky Team on Monday. On this team is a nurse who has been with us for 14 years. Never once in those 14 years has she been timely with her documentation.(I am not exaggerating!) She came to me on Wednesday and said she had all her patients seen and the documentation completed. She had even completed her Worksheets and Referral Agreement/POC for next Wednesday's Team Meeting. That was an amazing success!!! Betsy
